The undersigned confirms that the above information is true and accurate. In addition, they authorize Econolease Financial Services Inc., Chef Choice Equipment Rentals Inc., Services Financier Econolease and EFS Merchant Solutions Corp. (hereby referred to collectively as the “Econolease”), or any other financial institution referred in the present application, to request, use and share any information regarding the undersigned’s credit performance. The undersigned expressly consents, on its own behalf and, where applicable, on behalf of its directors, officers, shareholders, partners, guarantors, and authorized representatives, to the collection, use and disclosure by Econolease of personal information, including financial and credit information, for the purposes of assessing creditworthiness, processing and administering financing or credit applications, managing the commercial business relationship (including account administration, payment processing, risk monitoring, fraud prevention, and collection activities), and complying with applicable legal and regulatory obligations, including to and from credit bureaus and other third parties in order to assess the undersigned’s eligibility to Econolease’s financing and the terms applicable to such financing (the “Purpose”).
In addition, they authorize Econolease Financial Services Inc., Chef Choice Equipment Rentals Inc., Services Financier Econolease and EFS Merchant Solutions Corp. (hereby referred to collectively as the “Econolease”), or any other financial institution referred in the present application, to request, use and share any information regarding the undersigned’s credit performance, The undersigned acknowledges that Econolease, or any other financial institution referred in the present application may use automated systems, models, and technology enabled processes to assess the application and determine eligibility for financing products. Such collection and use of information is necessary for said parties to operate its business and provide financing.
The undersigned acknowledges that Econolease has a serious and legitimate interest in collecting such personal information, directly from the undersigned or from third parties, and that this collection and subsequent processing of information enables the undersigned to operate its business. The undersigned acknowledges that certain information may be collected, used, or disclosed without consent where permitted or required by applicable law.
The undersigned acknowledges that Econolease, or any other financial institution referred in the present application, may be subject the Proceeds of Crime (Money Laundering) and Terrorist Financing Act and its regulations. The undersigned agrees to provide true, accurate, current and complete information and documentation, and to promptly update such information upon request or upon any material change. Such information may include, without limitation, information required to verify the identity of the undersigned and its authorized representatives, identify any individual who directly or indirectly owns or controls 25% or more of the undersigned, identify any third party instructing or controlling the undersigned or the transaction, determine the source of funds, and assess whether the undersigned or any related person is a politically exposed person (PEP) or subject to economic sanctions. Where required by law, Econolease may collect and use this information without the undersigned’s consent.
The undersigned acknowledges that Econolease can establish a file with the personal information collected in this Credit Application and in accordance with the above (the “Credit Application File”) and that such Credit Application File will be processed in furtherance of the Purpose and in accordance with the Econolease Privacy Policy accessible at: https://www.econolease.com/privacy-policy/. The undersigned will have the right to access and rectify their personal information, and withdraw their consent, subject to applicable legal, regulatory and/or contractual requirements, or require additional information regarding such processing of personal information by sending a written request by email to: [email protected]. The undersigned also acknowledges that Econolease may assign its rights hereunder, in whole or in part, and may disclose any relevant financial information or other material in connection with the granting of such assignment, in accordance with applicable laws. This consent may be withdrawn at any time, by written communication addressed to Econolease.
